599 CHIP

Heritage Health (Medicaid):

599 CHIP members are limited to care solely for the unborn child, including prenatal care, professional fees for labor and delivery, live birth, fetal death, miscarriage, and ectopic pregnancy. Services not covered under 599 CHIP include postpartum care, medical issues unrelated to the pregnancy, and any services to the newborn child. 

599 CHIP prenatal visits and delivery services must be billed under a bundled rate methodology. Providers billing for prenatal visits and delivery services must bill under the following codes and modifiers:

59400 U1 Routine OB - antepartum and vaginal delivery

59510 U1 Routine OB - antepartum and cesarean delivery

59610 U1 Routine OB - antepartum and vaginal delivery after previous previous cesarean delivery

On the Health Insurance Claim Form:

  • In Box 2, list the patient name as the mother's name 
  • In box 1A, include the member ID number of the unborn child as found on their member ID card.  

If the patient is seen by a different provider for antepartum and delivery care, each provider will bill for the services that were rendered.

At the first visit the Notification of Pregnancy (PDF) form should be competed and faxed to 1-866-681-5125. This will provide Nebraska Total Care with the information needed for billing.
